1. General information

1.1. Aims of the course

The subject of Periodontology I must train the student for the diagnosis and treatment of periodontal diseases, as well as promote the development of preventive attitudes of such diseases.

For this reason, the course aims for the student to achieve:

  • Identify the presence of periodontal disease in the patient.
  • Establish a proper diagnosis of it.
  • Establish a general and individual prognosis for each of the pieces present in the mouth.
  • Plan the most appropriate periodontal treatment for each of the patients.
  • Carry out the established treatment plan and refer to a specialist if he considers that he is not capable of solving it alone, justifying why.
  • Practice a follow-up of each one of the patients and insist on preventive practices to avoid the establishment of the disease in healthy patients.

These proposals and goals are aligned with the following Sustainable Development Goals (SDGs) of the 2030 Agenda of the United Nations (https://www.un.org/sustainabledevelopment/), in such a way that the acquisition of the learning outcomes of the course provide training and competence to contribute to their achievement:

1.2. Context and importance of this course in the degree

There is no doubt that periodontal disease is one of the most prevalent diseases in the world. Some epidemiological studies consider it the second most common disease in the population. Hence the importance that this subject represents within health in general, and dentistry in particular, since it is related to other pathologies such as diabetes, cardiovascular diseases, arthritis, pregnancy, etc.

In this subject we will see what types of pathologies include periodontal diseases, how to establish the appropriate diagnosis for each of them, what relationship it has with the rest of the disciplines of dentistry such as, for example, endodontics, prosthodontics, oral medicine, conservative dentistry, implants, etc. and also how to face and carry out the pertinent treatment in each of the different situations.

2. Learning goals

2.1. Competences

- Perform basic treatments of oral and dental pathology in patients of all ages. Therapeutic procedures should be based on the concept of minimal invasion and on a global and integrated approach to oral and dental treatment.

- To diagnose, plan and carry out, in general, a multidisciplinary, sequential and integrated treatment of limited complexity in patients of all ages and conditions and in patients with special needs (diabetic, hypertensive, oncological, transplanted, immunosuppressed, anticoagulated, among others) or disabled.

- To establish said diagnosis and treatment plan, the dentist must acquire the following skills:

  • Taking and interpreting radiographs and other image-based procedures relevant to dental practice.
  • Make diagnostic models, mount them and take inter-occlusal records.
  • Determine and identify the aesthetic requirements of the patient and the possibilities of satisfying her concerns.
  • Identify the patient who requires special care, recognizing their characteristics and peculiarities.
  • Assess the motor and sensory function of the mouth, maxillae and adnexa, and perform limited procedures of invasive diagnostic techniques in soft tissues (biopsies).

- For the establishment of an adequate treatment, the dentist must be competent in:

  • Appropriate prescription of drugs, knowing their contraindications, interactions, systemic effects and repercussions on other organs.
  • Apply loco-regional anesthesia techniques.
  • Manage acute infections, including drug prescription and simple surgical aspects.
  • Identify and attend to any dental emergency.
  • CTreat both pharmacologically and surgically inflammatory processes of periodontal and / or peri-implant tissues including supra and subgingival periodontal instrumentation techniques.

Transversal competences:

  1. Know how to identify the concerns and expectations of the patient, as well as communicate effectively and clearly, both orally and in writing, with patients, family members, the media and other professionals.
  2. Understand and recognize the social and psychological aspects relevant to the treatment of patients.
  3. Understand the importance of maintaining and using records with patient information for subsequent analysis, preserving the confidentiality of the data.
  4. To know the general processes of the disease, which include infection, inflammation, alterations of the immune system, degeneration, neoplasia, metabolic alterations and genetic disorders.
  5. Be familiar with the general pathological characteristics of diseases and disorders that affect the organ systems, specifically those that have oral repercussions.
  6. Understand the fundamentals of action, indications and efficacy of drugs and other therapeutic interventions, knowing their contraindications, interactions, systemic effects and interactions on other organs, based on the available scientific evidence.
  7. Understand and recognize the principles of ergonomics and safety at work (including cross infection control, radiation protection, and occupational and biological diseases).
  8. Have the ability to make an initial diagnostic judgment and establish a reasoned diagnostic strategy, being competent in the recognition of situations that require urgent dental care.
  9. Establish the diagnosis, prognosis and adequate therapeutic planning in all clinical areas of Dentistry, being competent in the diagnosis, prognosis and preparation of the dental treatment plan of the patient who requires special care, including medically compromised patients (such as diabetics, hypertensive , immunosuppressed, anticoagulated, among others) and patients with disabilities.
  10. Propose and propose the appropriate preventive measures for each clinical situation.
  11. Recognize the role of the dentist in prevention and protection actions against oral diseases, as well as in the maintenance and promotion of health, both at the individual and community level.

2.2. Learning goals

To pass this subject, the student must demonstrate the following results:

1) Identify the symptoms referred by the patient, as well as the signs (through clinical examination and complementary tests), typical of periodontal pathology and peri-implant tissues.

2) Carry out a correct diagnosis, prognosis and treatment plan for the patient with a level similar to that worked in class.

3) Know how to distinguish all those situations that due to their complexity cannot be treated by the student, justifying why and know how to refer them to the corresponding specialist.

4) Carry out the basic phase of periodontal treatment, as well as the maintenance phase at a basic level.

5) Know how to properly assess the patient's conditions (eg age, sociocultural level, medical risks ...) in relation to the proposed treatment in order to satisfy her expectations, always assessing risks and benefits of the therapeutic act.

6) Know how to plan a work session in the dental office according to the functions that belong to each type of professional (dentist, clinical assistant, administrative).

7) Establish the therapeutic sequence in the event that a multidisciplinary treatment is required and know how to prepare an inter-consultation document in order to obtain the necessary information to be able to carry out the indicated treatment.

2.3. Importance of learning goals

The learning results obtained in the subject are important since they contribute in a fundamental way to attaining the knowledge and skills necessary for the diagnosis of periodontal diseases and to make the patient aware of the appropriate treatment for each case. Along with this, it will allow the student to carry out the basic treatment of periodontal diseases, which is considered fundamental in the Dentistry degree and is thus expressed in his Degree Project.

3. Assessment (1st and 2nd call)

3.1. Assessment tasks (description of tasks, marking system and assessment criteria)

- Objective test of knowledge. It is a final exam with multiple choice questions (40 in total with five options being only one correct answer and 4 reserve questions). To pass the test, a minimum of 28 questions must be answered correctly. Wrong answers do not subtract points, blank answers neither add nor subtract marks. This objective test will contribute 50% to the final grade.

- Carrying out a coursework and its defense: A topic will be randomly assigned. The coursework will have to be defended. The delivery date of the coursework and the date of defense will be indicated by the teacher at the beginning of the course.  This evaluation activity will contribute 20% to the final grade, taking into account that the work part and the defense part must pass independently, being 10% each of them. It is surpassed with a 5 out of 10.

- Carrying out practical exercises and assessment of technical skills developed in preclinical practices. This evaluation activity will contribute 30% to the final grade and is exceeded with a 5 out of 10.

Requirements to pass the course: It will be necessary to have passed all the assessment tasks of the course. In the case of not pass one of them, they must be done on the official date. The grades obtained in the rest of the tests will be kept for the same academic year.

Qualification system: The numerical qualification will be expressed in accordance with the provisions of art. 5.2 of Royal Decree 1125/2003 of September 5 (BOE September 18), which establishes the European credit system and the system of qualifications in official university degrees and valid throughout the national territory. Thus, the grades will be established in the following range: From 0 to 4.9: Fail (S); from 5.0 to 6.9: Pass (A); 7.0 to 8.9: Notable (N); from 9.0 to 10: Excellent (SB). The mention Honors may be awarded to students who have obtained a grade equal to or greater than 9.0.

1. Información Básica

1.1. Objetivos de la asignatura

La asignatura y sus resultados previstos responden a los siguientes planteamientos y objetivos:

La asignatura de Periodoncia debe de capacitar al alumno para el diagnóstico y tratamiento de las enfermedades periodontales, así como potenciar el desarrollo de actitudes preventivas de tales enfermedades.

Por ello, la asignatura pretende que el alumno consiga:

  • Identificar la presencia de enfermedad periodontal en el paciente.
  • Establecer un diagnóstico adecuado de la misma.
  • Establecer un pronóstico general del caso e individual por cada una de las piezas presentes en boca.
  • Planificar el tratamiento periodontal más adecuado para cada uno de los pacientes.
  • Realizar el plan de tratamiento establecido y derivar a un especialista si considera que no está capacitado para resolverlo solo, justificando por qué.
  • Practicar un seguimiento de cada uno de los pacientes e insistir en prácticas preventivas para evitar el establecimiento de la enfermedad en pacientes sanos.

Estos planteamientos y objetivos están alineados con los siguientes Objetivos de Desarrollo Sostenible (ODS) de la Agenda 2030 de Naciones Unidas (https://www.un.org/sustainabledevelopment/es/), de tal manera que la adquisición de los resultados de aprendizaje de la asignatura proporciona capacitación y competencia para contribuir en cierta medida a su logro: 

  • Objetivo 3: Salud y bienestar.  

1.2. Contexto y sentido de la asignatura en la titulación

No cabe duda de que la enfermedad periodontal es una de las enfermedades con mayor prevalencia en el mundo. Algunos estudios epidemiológicos la consideran la segunda enfermedad más común en la población. De ahí la importancia que representa esta materia dentro de la salud en general, y de la odontología en particular, puesto que tiene relación con otras patologías como la diabetes, las enfermedades cardiovasculares, la artritis, el embarazo, etc.

En esta asignatura veremos qué tipo de patologías engloban las enfermedades periodontales, cómo poder establecer el diagnóstico adecuado a cada una de ellas, qué relación tiene con el resto de disciplinas de la odontología como por ejemplo, la endodoncia, la prostodoncia, la medicina oral, la odontología conservadora, los implantes, etc. y además cómo afrontar y realizar el tratamiento pertinente ante cada una de las diferentes situaciones.

2. Competencias y resultados de aprendizaje

2.1. Competencias

Al superar la asignatura, el estudiante será más competente para...

Competencias especificas:

CE.MIV.1. Realizar tratamientos básicos de la patología buco-dentaria en pacientes de todas las edades. Los procedimientos terapéuticos deberán basarse en el concepto de invasión mínima y en un enfoque global e integrado del tratamiento buco-dental.

CE.MIV.2. Diagnosticar, planificar y realizar, con carácter general, un tratamiento multidisciplinar, secuencial e integrado de complejidad limitada en pacientes de todas las edades y condiciones y en pacientes con necesidades especiales (diabéticos, hipertensos, oncológicos, trasplantados, inmunodeprimidos, anticoagulados, entre otros) o discapacitados. Específicamente,  debe ser competente en el establecimiento de un diagnóstico, de un pronóstico y el desarrollo de una adecuada planificación terapéutica en: CE.MIV.2.4. Patología periodontal y de los tejidos peri-implantarios;

CE.MIV.3. Para el establecimiento de dicho diagnóstico y plan de tratamiento el dentista debe adquirir las siguientes competencias:

CE.MIV.3.1. Tomar e interpretar radiografías y otros procedimientos basados en la imagen, relevantes en la práctica odontológica;

CE.MIV.3.2. Realizar modelos diagnósticos, montarlos y tomar registros inter-oclusales;

CE.MIV.3.3. Determinar e identificar los requisitos estéticos del paciente y de las posibilidades de satisfacer sus inquietudes;

CE.MIV.3.4. Identificar al paciente que requiera cuidados especiales, reconociendo sus características y peculiaridades;

CE.MIV.3.5. Valorar la función motora y sensorial de la boca, los maxilares y anejos, y realizar procedimientos limitados de técnicas diagnósticas invasivas en tejidos blandos (biopsias).

CE.MIV.4. Para el establecimiento de un adecuado tratamiento el dentista debe ser competente en:

CE.MIV.4.1. Prescripción apropiada de fármacos, conociendo sus contraindicaciones, interacciones, efectos sistémicos y repercusiones sobre otros órganos;

CE.MIV.4.2.  Aplicar técnicas de anestesia loco-regional;

CE.MIV.4.5. Manejar infecciones agudas, incluyendo la prescripción farmacológica y los aspectos quirúrgicos sencillos;

CE.MIV.4.6.  Identificar y atender cualquier urgencia odontológica;

CE.MIV.4.10. Tratar tanto farmacológica, como quirúrgicamente los procesos inflamatorios de los tejidos periodontales y/o peri-implantarios incluyendo las técnicas de instrumentación periodontal supra y subgingival;

Competencias transversales:

CG.3. Saber identificar las inquietudes y expectativas del paciente, así como comunicarse de forma efectiva y clara, tanto de forma oral como escrita, con los pacientes, los familiares, los medios de comunicación y otros profesionales.

CG.4. Comprender y reconocer los aspectos sociales y psicológicos relevantes al tratamiento de pacientes.

CG.9. Comprender la importancia de mantener y utilizar los registros con información del paciente para su posterior análisis, preservando la confidencialidad de los datos.

CG.14. Conocer de los procesos generales de la enfermedad, entre los que se incluyen la infección, la inflamación, las alteraciones del sistema inmune, la degeneración, la neoplasia, las alteraciones metabólicas y los desórdenes genéticos.

CG.15. Estar familiarizado con las características patológicas generales de las enfermedades y trastornos que afectan a los sistemas orgánicos, específicamente aquellas que tienen repercusión bucal.

CG.16. Comprender los fundamentos de acción, indicaciones y eficacia de los fármacos y otras intervenciones terapéuticas, conociendo sus contraindicaciones, interacciones, efectos sistémicos e interacciones sobre otros órganos, basándose en la evidencia científica disponible.

CG.17. Comprender y reconocer los principios de ergonomía y seguridad en el trabajo (incluyendo control de infecciones cruzadas, protección radiológica y enfermedades ocupacionales y biológicas).

CG.22. Tener capacidad para elaborar un juicio diagnóstico inicial y establecer una estrategia diagnóstica razonada, siendo competente en el reconocimiento de las situaciones que requieran una atención odontológica urgente.

CG.23. Establecer el diagnóstico, pronóstico y una adecuada planificación terapéutica en todas las áreas clínicas de la Odontología, siendo competente en el diagnóstico, pronóstico y elaboración del plan de tratamiento odontológico del paciente que requiera cuidados especiales, incluidos los pacientes médicamente comprometidos (como diabéticos, hipertensos, inmunodeprimidos, anticoagulados, entre otros) y pacientes con discapacidad.

CG.27. Plantear y proponer las medidas preventivas adecuadas a cada situación clínica.

CG.30. Reconocer el papel del dentista en las acciones de prevención y protección ante enfermedades bucales, así como en el mantenimiento y promoción de la salud, tanto a nivel individual como comunitario.

2.2. Resultados de aprendizaje

El estudiante, para superar esta asignatura, deberá demostrar los siguientes resultados...

1) Identificar los síntomas referidos por el paciente así como los signos (mediante la exploración clínica y las pruebas complementarias), propios de la patología periodontal y de los tejidos periimplantarios. 

2) Realizar un correcto diagnóstico, pronóstico y plan de tratamiento del paciente con un nivel similar al trabajado en la clase.

3) Saber distinguir todas aquellas situaciones que por su complejidad no pueden ser tratadas por el alumno justificando el por qué y,  saber referirlas al especialista correspondiente. 

4) Realizar la fase básica del tratamiento periodontal así como la fase de mantenimiento a un nivel básico.

5) Saber valorar adecuadamente las condiciones del paciente (ej. edad, nivel sociocultural, riesgos médicos…) en relación al tratamiento propuesto a fin de poder satisfacer sus expectativas, siempre valorando riesgos y beneficios del acto terapéutico.

6) Saber planificar una sesión de trabajo en el gabinete dental según las funciones que pertenecen a cada tipo de profesional (odontólogo, auxiliar de clínica, administrativo). 

7) Establecer la secuencia terapéutica en el caso de que se requiera un tratamiento multidisciplinario y saber elaborar un documento de interconsulta con el objetivo de conseguir la información necesaria para poder llevar a cabo el tratamiento indicado.

2.3. Importancia de los resultados de aprendizaje

Los resultados de aprendizaje que se obtienen en la asignatura son importantes ya que contribuyen de manera fundamental a alcanzar los conocimientos y las habilidades necesarias para el diagnostico de las enfermedades periodontales y dar a conocer al paciente el tratamiento adecuado para cada caso. Junto con esto, permitirá al alumno a llevar a cabo el tratamiento básico de las enfermedades periodontales, lo que se considera fundamental en la titulación de Odontología y así queda expresado en su Proyecto de Titulación.

3. Evaluación

3.1. Tipo de pruebas y su valor sobre la nota final y criterios de evaluación para cada prueba

El estudiante deberá demostrar que ha alcanzado los resultados de aprendizaje previstos mediante las siguientes actividades de evaluación:

  • Prueba objetiva de conocimiento. Se trata de un examen final con preguntas tipo test (40 en total con cinco opciones siendo solo una respuesta la correcta y 4 preguntas de reserva). Para superar la prueba hay que contestar correctamente un mínimo de 28 preguntas. Las respuestas erróneas no restan puntos, las respuestas en blanco ni suman ni restan puntuación. Esta prueba objetiva contribuirá un 50% a la calificación final.

  • Realización de trabajo y su defensa: el profesorado asignará un tema de forma aleatoria para la realización del trabajo, que tendrá que ser defendido. La fecha de la entrega como de la defensa del mismo será indicada por el profesorado al comienzo de curso. Supondrá un 20% de la nota global teniendo que estar aprobada la parte del trabajo y la parte de la defensa de forma independiente, siendo un 10% cada una de ellas. Se supera con un 5 sobre 10.

  • Realización de ejercicios prácticos y valoración de la destreza técnica desarrollada en las prácticas preclínicas. Esta actividad de evaluación contribuirá un 30% a la calificación final y se supera con un 5 sobre 10.

Requisitos para superar la asignatura: Será necesario tener superadas todas las partes de la asignatura. En el caso de no superar una de las pruebas, deberá recuperarse en la fecha oficial de la asignatura, guardándose las calificaciones obtenidas en el resto de las pruebas para el mismo curso académico.

SISTEMA DE CALIFICACIONES:

La calificación numérica se expresará de conformidad con lo establecido en el art. 5.2 del Real Decreto 1125/2003 de 5 de septiembre (BOE 18 de septiembre), por el que se establece el sistema europeo de créditos y el sistema de calificaciones en las titulaciones universitarias de carácter oficial y validez en todo el territorio nacional.

Así, las calificaciones se establecerán en el siguiente rango: De 0 a 4,9: Suspenso (S); de 5,0 a 6,9: Aprobado (A); de 7,0 a 8,9: Notable (N); de 9,0 a 10: Sobresaliente (SB). La mención Matrícula de honor podrá ser otorgada a alumnos que hayan obtenido una calificación igual o superior a 9,0.

El proceso de aprendizaje que se ha diseñado para esta asignatura se basa en lo siguiente:
